When your care requires a specialist, we do not just hand you a name and send you on your way. We coordinate the entire process, transferring your records, helping with scheduling, and making sure the specialist has the full picture before you walk through their door. The Southcoast is home to excellent providers and we know who to trust for your specific needs.

After your specialist visit, we follow up to review recommendations and adjust your care plan accordingly. You should never feel like you are bouncing between providers without anyone connecting the dots. We stay involved the entire time so your care remains seamless and nothing important gets lost along the way.
